(1) The division shall provide for the evaluation of any person eligible for diagnostic and evaluation services either through services funded by the department or by referral to another agency.

(2) Within 30 calendar days of the enrollment of a developmentally disabled person in a provider service program, with the exception of respite and transportation services, the provider shall perform a comprehensive skill assessment for that person. Each assessment shall be reviewed annually by the provider. Results of the assessment shall be provided to the client's individual habilitation planning team, including the individual receiving services.
