37.86.3905    CASE MANAGEMENT SERVICES FOR CHILDREN WITH SPECIAL HEALTH CARE NEEDS, COVERAGE

(1) The following services are reimbursable case management services for children with special health care needs:

(a) assessment;

(b) case planning;

(c) care coordination and referral; and

(d) monitoring.
