(1) The license, together with evidence of current validation, shall be conspicuously displayed within the facility served by the licensee and the holder of a license shall have evidence of current validation in his possession at all times.

(2) Each person who holds a license or permit shall file his mailing address with and notify the board in writing of any and all changes which occur within ten days of such change.
