24.29.801    ACCIDENT REPORTING

(1) Upon notice of an accident, injury, or occupational disease an employer shall, within six days of such notice, submit to the employer's workers' compensation insurer or to the department, a completed form known as the first report of occupational injury/occupational disease.

(2) If an insurer changes an adjuster or an employer changes its third party adjuster, the department must be notified in writing by the office assuming the account(s) , within 30 days from the time the account is transferred. The notification must include:

(a) the adjuster = s or third party adjuster's business address and telephone number;

(b) an effective date of transfer;

(c) a general description of the type of claims transferred (if there are identifiable categories or classes of claims) ; and

(d) upon request for each claim transferred:

(i) the claimant = s name;

(ii) the claimant = s social security number;

(iii) the date of injury or occurrence; and

(iv) the insurer = s claim number.
