(1) Application fee                                                                            $125.00

(2) Renewal fee (based on annual renewal)                                      110.00

(3) Renewal fee (inactive to active)                                                      55.00

(4) Inactive certificate fee (based on annual renewal)                          55.00

(5) Additional standardized fees are specified in ARM 24.101.403.
