2.43.605    DESIGNATION OF BENEFICIARY

(1) The participant shall make the selection of beneficiary, in writing and on the form provided by the division for this purpose, dated and signed by the participant and witnessed by a disinterested third party who shall attest to the voluntary nature of the participant's action.

(2) The designation of beneficiary shall be effective immediately upon receipt by the division.
