2.63.607    BUSINESS CHANGES

(1) A licensee must notify the director of any of the following changes in the licensed business:

(a) any change of business structure;

(b) material change of ownership;

(c) voluntary or involuntary business closure;

(d) changes in the officers;

(e) changes in the board of directors; and

(f) change of business address.
