(1) Programs will be licensed annually. Beginning July 1, 2009, the annual period will run from July 1 to June 30 of the following year.

(2) Licensing fees are based on the program's average daily census at the time of application and are paid at 20% of the combined registration and licensing fees:

(a) 0-10 participants                                               $ 270

(b) 11-25 participants                                               695

(c) 26-50 participants                                             1300

(d) 51-100 participants                                           2130

(e) 101 and more participants

Programs with 101 and more participants must contact the board for current information on the average daily census rate to determine the licensing fee balance.

(3) All fees provided for in this rule are nonrefundable and are not prorated for portions of the licensing period.
