(1) All programs must register prior to being licensed.

(2) The registration fee covers a one year period.

(3) Registration fees are calculated by each program being assessed a base fee of $500 to which is added an amount based on the program's average daily census. Registration fees are collected at the time of application and allow an applicant to apply for a provisional license.

(a) 0-10 participants $ 1080

(b) 11-25 participants 2780

(c) 26-50 participants 5210

(d) 51-100 participants 8520

(e) 101 and more participants

Programs with 101 and more participants must submit the base fee and contact the board for current information on the average daily census rate to determine the registration fee balance.

(4) For any program registering at any time between July 1, 2008, and June 30, 2009, such program shall pay both their registration fee and their provisional licensing fee at the same time. The provisional licensing fee shall cover the period up to and including June 30, 2009.

(5) Any program that is registered and/or licensed (provisional or otherwise) during the period between July 1, 2008, and June 30, 2009, will be required to renew the license on or before June 30, 2009.

(6) All fees provided for in this rule are nonrefundable and are not prorated for portions of the registration period.
