2.43.402    MEMBERSHIP CARDS

(1) Each contributing employee must complete a membership card upon employment, name change or change of beneficiary, and this card must be forwarded by the employing agency to the retirement division.

(2) No benefit will be processed, or refund of contributions made, unless the division has a completed membership card on file.
