(1) An application to permit an electronic video gambling machine must be submitted to the Gambling Control Division of the Department of Justice upon forms prescribed by the department. Form 8 is a video gambling machine permit application; Form 8 is available upon request from the department. The application is not complete unless it is dated and signed by the applicant, and contains all information and statements required by the department.

(2) A separate application must be completed for each machine.

(3) The permit fee required by 23-5-612 , MCA, must accompany each permit application unless the permit application is for conversion of a permitted video gambling machine to multi-game and an agreement (Form 31) has been executed to commit the video gambling machine to be connected to the automated accounting and reporting system.

(4) A machine owner who utilizes an approved accounting and reporting system may submit permit applications through an internet web site provided by the department.
