(1) An inpatient free standing, intensive outpatient or intermediate care provider may apply for enrollment as a medicaid provider of substance dependency and abuse treatment services.

(2) The enrollment standards and procedures set forth in the chemical dependency bureau manual entitled: "Explanation of CD Medicaid and State-Paid Services, Instructions for Determining Eligibility for Services and Billing Under Bureau Contracts" dated September 2002, are hereby adopted and incorporated by reference. (3) To be enrolled, a provider must be approved and must be under contract with the chemical dependency bureau to provide substance dependency and abuse services.
